What is home health care?

Home health care consists of a wide range of health care services that can be given in your home for an illness or injury. Home health care consists of Skilled Nursing, Certified Home Health Aides, Physical Therapy, Occupational Therapy, Speech Therapy and Medical Social Services. These services are administered in the patient’s home by professional home health care staff.

Guidance Home Health, in collaboration with the client’s physician, will develop a specific Plan of Care specifically made to meet the individual needs of the client.

Who is eligible for home health services?

Clients with limited physical abilities that wish to stay in their home environment while receiving medical treatment are eligible for home health. Medicare guidelines require:

  • A physician’s order and a skilled nursing need in addition to homebound status.

  • You must need one or more of the following skilled needs that require the attention of a professional: skilled nursing care, physical therapy, occupational therapy, or speech therapy.

  • The home care agency caring for you must be Medicare-certified.

  • Homebound does not mean “housebound” but leaving the home takes considerable and taxing effort and the patient must require assistance to leave the home. 

Medicare pays for 100% of home care services and private insurance companies may cover a range of home care services. For Medicare, the patient will not be billed. If a patient only has private insurance, a patient might be billed a portion of the fee not covered by the insurance plan. You will be notified if there is a portion that will not be covered by your insurance plan or Medicare upon admission.

How do you get home health services?

Home Health is prescribed by the Patient’s doctor.

Your physician will call or fax us an order, or prescription, for home care services that identifies which services are indicated. We will have the admitting clinician call and set up an appointment time that is agreeable to perform your admission. You may also contact us directly and we can coordinate services with your doctor.

The patient’s specific Plan of Care will be monitored regularly and any changes in condition will be communicated immediately to your physician.
